After clicking Create a New Patient, you will see a blank Patient Details screen (shown below):

At this point, you should determine whether your patient has had a Risk Assessment in the past. If so, there will be an established PreViser ID for this patient that includes all previous Risk Assessments which you can access.

If the patient has not had a PreViser Risk Assessment before:

1. Skip the PreViser ID field at the top. Fill out the relevant required fields on the form (marked with a green asterisk).

2. Click Save to store all the information for that patient’s record and return to the Risk Assessment Home screen. (To save this record, you can either click the text Save for Later in the upper right, or the green Save button at the bottom of the screen.)

3. When you Save the new record, a unique PreViser ID is automatically generated and assigned to that patient. (See PreViser ID in the Definition of Terms in the Appendix for more information).

On the next page, we will look at what to do if the patient has had a PreViser Risk Assessment before.
