| A Treatment Option Plan is a visual report of the information gathered and calculated from either of the three types of Risk Assessment (either Perio, Caries, or Oral Cancer). Each type of Risk Assessment produces a report that may be viewed in either one or two design formats, including the same information but designed for two different purposes: |

| The following is a list of all the possible report formats. (There are a variety of Caries reports targeted to a specific age range as indicated. Note that currently, Oral Cancer Risk Assessments only produce a report in Patient Format.) |
