The Input
Forms are formatted so you can print them and easily record all key data
you will need for the Risk Assessment. Note that the forms in the links
above are only samples; you should access the correct forms directly from
the application's Home page, rather than this User's
Guide.
Each form
requests slightly different information, but generally you need to indicate
the following:
Patient
Info:
Identifying information about the individual the report is prepared for
and about, including the Name, Date of birth (which determines the age
on the exam date), Patient ID and PreViser ID. (Those two ID's are part
of the patient's records in your local PreViser files, so you don't need
to obtain them from the patient)
Patient History and Clinical Data:
Information related to the patient's health history and habits (e.g. smoking,
oral hygiene), and clinical conditions (observing the presence of bleeding,
furcation involvements, etc.)
Perio
Measurements:
For a Perio form, you need to indicate data on pocket depths and radiographic
bone height from the CEJ, for each quadrant.
Target
Tooth:
On Caries forms for younger patients, you are asked to indicate the target
tooth (the tooth most recently exposed to saliva for the last 12 months),
and mark any conditions that are true for those teeth.
Treatments Performed since the last Risk Assessment:
Four main headings with items listed under each, asking for a
numerical response if applicable. The form asks for the date of the last
Risk Assessment so that you can determine these answers correctly.
Note: This is relevant only for patients who
have had a previous Risk Assessment. If this is the patient's first Risk
Assessment, you can completely ignore this section on
the form, as you will not see that entry screen in the application.
