Risk Assessment Input Forms

To assist you in performing Risk Assessments, PreViser includes printable Risk and Disease Analysis Input Forms for your use in gathering and recording data from the Patient during the exam.

Each of the available forms prints on one page and collects the data required for input for either a Caries or Perio Risk Assessment. Since the RiskCalculator™ is designed to use data already gathered during a standard clinical exam, these forms may overlap or duplicate other information-recording systems you already use.

The benefit to using the PreViser form is knowing you have compiled all the items required to calculate a Risk Score. However, feel free to find whatever solution works best for your practice's routine.

The following forms are available:
  • Perio (all ages)
  • Caries (Ages 1-4)
  • Caries (Ages 5-8)
  • Caries (Ages 9-18)
  • Caries (Ages 19-33)
  • Caries (Ages 34 and over)
  • Oral Cancer (all ages)

You have two options in printing and using these forms:

1. You may print completely blank forms from the Home page.
2. You may print a form partially completed with information you have started to enter for a specific patient's Risk Assessment.

On the next page, we will look at the contents and samples of these forms, and then we will show the steps of printing each type of form.
